Application for Licensure

Nurse Licensure Compact

The Nurse Licensure Compact, adopted in Idaho on July 1, 2001, allows nurses who reside in and hold a license in one Compact state, the privilege of practicing nursing in other Compact states, without holding additional licenses in those states. Under terms of the Compact, nurses hold a valid license in only one Compact state – their primary state of residence. If you declare Idaho as your primary state of residence, you will be issued an Idaho license, and licenses that you currently hold in other compact states (i.e. Arizona, Arkansas, Colorado, Delaware, Iowa, Kentucky, Maine, Maryland, Mississippi, Missouri, Nebraska, New Hampshire, New Mexico, North Carolina, North Dakota, Rhode Island, South Carolina, South Dakota, Tennessee, Texas, Utah, Virginia, and Wisconsin) will become invalid. You are granted the privilege to practice in other compact states and are required to adhere to the practice laws of each respective state in which you practice. If you change your state of residence to another state, you are directed to contact that state’s Board of Nursing and be granted anew license. If your new state of residence is a member of the Compact, your Idaho license will be come invalid upon issuance of your new license. If your new state of residence is not a member of the Compact and you choose to continue practicing in Idaho, you will be issued an Idaho license designated as valid only for practice in Idaho.

Licensure by Endorsement

Requests for applications for licensure by endorsement may be submitted to the Board via telephone (208) 334-3110, fax (208) 334-3262, or mail (PO Box 83720, Boise, Idaho 83720-0061. If you request an application by one of these methods, please provide the following information with your request:

Name, Telephone Number
Mailing Address
Original State of Licensure
If you have had previous licensure in Idaho
If you need an application for RN, LPN or advanced practice professional nurse licensure (CNM, CNS, NP or RNA)

Application packets are available under "Forms". These forms are available in Adobe Portable Document Format (PDF). To view these forms you will need to have the Adobe Acrobat Reader installed on your computer. If you do not have this program, go to the AdobeExternal Link web site to download a free copy.  You will need to contact the Board office (208) 334-3110 ext 2476 for a Fingerprint Card.

Processing of application materials may take from 4-6 weeks; a temporary license is available upon receipt of the completed application, fee and appropriate materials.


Licensure by Examination

Requests for applications for licensure by examination may be submitted to the Board via telephone (208) 334-3110, fax (208) 334-3262, or mail (PO Box 83720, Boise, Idaho 83720-0061. If you request an application by one of these methods, please provide the following information with your request:

- Name, Telephone Number
- Mailing Address
- Date of Graduation
- School of Graduation
- Indicate if you need an application for the professional (RN) or practical nurse examination (PN)

If you are applying for examination by equivalent education, please contact the Board office to see if you qualify.

Nurse Aide Registry

The Board of Nursing does not have information related to the Nurse Aide Registry. If you are requesting the registry status of a nurse aide, applications for placement on the Registry, test schedules, test sites, qualifications for testing, or other questions related to the Registry, please contact the Idaho State Nurse Aide Registry at (800) 748-2480 (locally at 334-6620) or email IDNAP@idhw.state.id.us.
